For reasons of data security, treatment-related details and personal identity codes must not be sent with the feedback form or by e-mail. Your feedback will give us the information we need to make further improvements to our service. If you prefer, you can send your feedback anonymously.

Enquiries and detailed feedback related to treatment should be sent directly to the unit in question, either in writing or by telephone. Patient details and the booking and cancelling of appointments are not handled by e-mail or through the feedback form.
